Documentation Guidelines
Before you may request accommodations, documentation of the disability must be provided. Documentation should be as current as possible (typically within three years) and must be furnished by a licensed medical or psychological examiner on official letterhead and signed by the examiner. Reports by nurses, counselors, social workers, record clerks and statements written on prescription pads are not acceptable.

Emotional Support Animal
Students can submit a request for an Emotional Support Animal (ESA) to live with them in on-campus housing. ESAs should not be brought into other buildings on campus. The university has a housing committee who meets on requests like this. The committee consists of a representative from housing, the counseling center, health services, and our office. Once a decision is made, students are notified via Â鶹¹û¶³´«Ã½ email.
